Information on the processing of personal data
CONSENT TO THE COLLECTION AND PROCESSING OF PERSONAL DATA
in the sense of Act No. 101/2000 Coll., on the protection of personal data and Regulation (EU) 2016/679 of the European Parliament and of the Council
By submitting this form and "clicking" on the option I agree, as the subject of personal data, I give my consent to the processing of my personal data listed below, which I hereby voluntarily provide to the personal data administrator, in the manner specified further in this consent. I acknowledge that these personal data concerning me constitute so-called personal data within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons in connection with the processing of personal data and on the free movement of this data and on the repeal of Directive 95/46/EC ("GDPR"). At the same time, I hereby declare that I am at least 16 years old and, if not, I confirm that this consent has been expressed or approved by a person exercising parental responsibility towards me.
1. Administrator
The administrator of personal data is the company Spolmont, spol. s.r.o., with registered office at Jiráskova 381, 755 01 Vsetín, ID number: 64084990, registered in the commercial register maintained by the Regional Court in Ostrava, section C, file 8656 ("Administrator").
2. Processing of personal data
2.1. My personal data, which I voluntarily provide to the Personal Data Manager, are as follows: Name, Surname, E-mail, Telephone number.
2.2. I consent to the processing of my above-mentioned personal data for the purposes of:
the Administrator's marketing purposes associated with the offer of products and services; sending information about organized events, products, services and other activities (e.g. by sending newsletters, telemarketing); contacting for market research and marketing research purposes; contacting for the purpose of wishes for Christmas and Easter or other holidays and sending discount vouchers, gifts, etc.;
whereas the processing of personal data is necessary to fulfill these purposes.
2.3. The legal basis for processing is Article 6 paragraph 1 letter a) GDPR.
2.4. The Administrator processes personal data that are adequate, relevant and limited to the necessary scope in relation to the purpose for which they are processed.
2.5. In addition to the Administrator, the above personal data will be collected and processed by the following entities:

Recipients) of personal data: The Administrator is entitled to transfer the personal data of the data subject in particular to the following recipients and categories of recipients: suppliers of the Administrator, employees of the Administrator, providers of transport services, providers of marketing and advertising services, financial institutions, state authorities within the framework of the fulfillment of legal obligations established by relevant legal regulations and other persons in a different contractual relationship with the Administrator.

2.6. The Administrator is authorized to process the above-mentioned personal data for a period of 5 years from the granting of my consent. I understand that before the expiry of this period the Administrator may contact me to renew my consent and that without renewal my personal data will be permanently deleted after the expiry of the above period.
3. Rights of the data subject
3.1. I acknowledge that as a subject of personal data I have the right to request from the Controller access to my personal data (Article 15 GDPR), their correction or deletion (Article 16 or Article 17 GDPR), or restriction of processing (Article 18 GDPR) and the right to object to the processing carried out (Article 21 GDPR). Under the conditions set out in Article 20 of the GDPR, as a data subject, I have the right to the portability of personal data that concern me and that I have provided to the Controller.
3.2. As a data subject, I can contact the Administrator regarding the processing of my personal data at the e-mail address
3.3. If, as a data subject, I believe that my personal data is being processed in violation of legal regulations, I have the right to contact the Administrator with a request for rectification. If the request is found to be justified, the Administrator will immediately remove the objectionable condition. This does not affect my possibility as a data subject to file a complaint directly with the Office for Personal Data Protection.
3.4. I hereby give my consent to the processing of my personal data by the Administrator for the purposes, to the extent and for the period specified above. I understand that the Administrator will archive this consent for the purpose of fulfilling the Administrator's legal obligation to be able to prove that I have given consent to the processing being carried out.
